Because medical records are confidential, Central Jackson County FPD requires your written permission for the release of information, except in those situations specified by law. Please complete the Release of Information Form and either fax to

Financial Assistance
If you are unable to pay your medical bills because you are uninsured or under-insured, you may qualify for financial assistance under our financial assistance program. Click here for the Financial Hardship Application. If you have any questions or need further assistance with the completion of the application, contact the Central Jackson County FPD Billing Analyst at (816) 229-2522, Monday through Friday, 8:00 AM to 4:30 PM.
